When assessing a unit/plant/equipment/vehicle, a fitter might be checking for defects. Every time a defect is found, he needs to describe what the defect is, where he found it, and the severity of the defect (through its likelihood and consequence). The fitter is counting defects and collecting the same field measures each time he finds a defect.
This is what a Series is – a way to capture the same field measures multiple times. You might like to apply this to your risk assessment process. Each time you identify a risk, you need to describe it, then rate it, provide controls to mitigate it and then reassess the residual risk.
